The townspeople are outraged by the proposal to introduce a limit on free calls and outsource the ambulance in private hands

At the end of June the Ministry of health, by order of the President, announced a series of proposals, including providing for full operation in the country private organizations, offering patients an ambulance and emergency medical care on a commercial basis.
Show all… the statement of the Ministry met expectations partially. Free to seek the services of an emergency can be no more than four times a year – perhaps this is the main thing that have learned, citizens of many messages of various news agencies. This rule does not apply to the disabled, children and pensioners, and, nevertheless, the opinion of the residents of Birobidzhan on the potential of innovations has been mostly negative, IA ЕАОMedia citing an article by Alexander Drabkin “Right to assistance” in the newspaper “Di box”.

“Svetlana DOBROVA:

– This is a travesty! If a person calls an ambulance, so he really needs help and not have to think about, once again calling. Who is to count and decide met the challenge the norm or not? Better to let the state declare that anyone who is not a disabled child or a pensioner, the ambulance will be paid then we don’t need to count anything and anyone.

Elena GAVRILYUK:

– Once again we are deprived of confidence in the future, that if necessary, the doctors will always come to the rescue, how many times have not had to use it.

Vitaly EREMIN:

– We have all done “for the people”, the laws are such that the welfare of the people became outlaws. The time will come – and we will never be able to call an ambulance. It is not clear what are we paying taxes?..

TODAY in Birobidzhan hour 14 crews of emergency medical care. They are able to serve about 40,000 calls a year, far more than stipulated in current regulations.

Leonid GULENOK, chief physician ALGUS “Birobidzhan ambulance station”, the main in the field of freelance specialist in the specialty of “emergency medical care”, health working 48 years. Who but he can comment on the current situation and to predict what the prospects for future changes?

– The controversy about ambulance has been ongoing for several years – began Leonid Pavlovich. It was expected to adopt a special Federal law about it, and after all ended up being came out just the Order number 388Н “ON the procedure for the provision of emergency medical care.” In the past three years, he repeatedly refined, but almost remained “premature child.” Let’s start with the fact that the attendance of therapists at home in the region decreased by 60 percent. What has happened to these patients? They lay on the shoulders of the ambulance. We serve up to forty thousand calls per year, and payment to us is made only for 31 thousand. The remaining FOMS are not accepted, because Russia accepted norm of 318 calls per one thousand population. We have, given the characteristics of the region, allowed 330. Other people don’t get into the registry because not identified, do not have documents, leading asocial way of life… they also include ordinary citizens who, say, came to visit from the village, not having neither passports, nor the policy. By 20 June, the Ministry of health will report to the President their proposals based on Our conversation with Leonid Gulenko took place a week ago. – Approx. ed.) in regard to a private ambulance. Should be transferred to the ambulance for mode private partnerships with transportation companies. In some subjects of Federation are already trying to do this. But I always turn to the folk wisdom: “he Who pays the Piper calls the tune”. About five years ago on this topic refused Khabarovsk, where the car was serviced by the transportation company and came to help on call. Transport workers the chief physician did not obey. This is not justified. We, finally, have ensured that our machines are equipped with expensive equipment, which requires regular calibration. They have everything from blood pressure monitors and oxygen tanks to the cardiograph. Who, exactly, responsible?! At the station the machine adopts duty, checks the completeness and maintenance of equipment, and who will do it in the Park? And storage of medications, care, treatment?..

The state’s DESIRE to get rid of a burden in the form of transport emergency is clear. If cars with red crosses on the sides will own the private traders, of trouble the officials will be much less. The technical Park of the doctors and so are not particularly worried the authorities. To date, the wear and tear of vehicles Birobidzhan ambulance station is 85%. If I can fix the situation for private motor transport enterprise? Be aware that ambulances must be managed by most skilled and therefore highly paid drivers. Now in return, they get certain allowances. Will this entrepreneurs? Such questions, I asked one of them, the owner of the bus Park of Vladimir, BOLVANENKO.

– If we would pay the money – said Vladimir Nikolaevich – the work we will be able to perform any, including servicing of the fleet. Another thing, do we really need ambulances were separate from the station? Doctors, nurses, paramedics and drivers should be as they were, a single organism, to understand each other. It is unclear why break something that already worked for years and does not fail.

As if to confirm the above, Leonid Gulenok again argues, based on his own practice:

For 18 years of my work there was not a single day failure of the visiting teams. If machines break down, drivers go out in my spare time and repair them. Spare parts we acquire for the account of paid services: removal of corpses, transportation of patients, examination of drivers, injection, physiotherapy… Those who are in the experiment implemented the experience of working with private partners, has acquired a lot of problems. Drivers seriously lost wages, they even strikes carried out…

I am SURE that if the health workers received the command to transfer the ambulances to the private sector, to fight against this decision no one will. Left to float freely, settling in the premises of state medical institutions, medical laboratories, are not empty private diagnostic centers and clinics. But the private ambulance?.. I still think that it is somewhat different. Will it be citizens, whether able to pay for its services?

In Moscow, for example, call a private ambulance within the Moscow ring road is 5 thousand rubles, abroad – 6 thousand. In Yaroslavl the challenge within the city costs 2.9 thousand rubles for the city’s 3.4.

– Of course, the option of private ambulance work possible, – concluded Leonid Pavlovich. But here we are talking about how to create an ambulance on the basis of public medical institutions. They will benefit from our control room to receive calls, but their work is mostly chronic and touches, mind you, paying patients, who in the city not so much. Emergency calls will still have to serve us because one or two private brigade can not cope with them. And we will remain the attics, basements, and all social sites. Result such example: managers already know people who call an ambulance for twenty times a month, because suffer from chronic diseases. One of the patients have an accurate diagnosis, she’s prescribed treatment. But she calls us crying into the phone and said that she had no money to buy medicine! And we go to her, knowing that the call we will not pay. Begs the sad conclusion: if the government decides that people are not in the category of disabled, pensioners and children will be able to call an ambulance not more than four times a year, this will lead to cuts in funding, reduction in staff. In the end we may be left two or three brigades. We are, even if you want to not be able to help many people who though are at the bottom of society, but not deprived of their constitutional rights to health protection and medical care.

The Ministry of healthcare reports
20 Jun, 17:05 Moscow time on the official website of the Ministry of health of the Russian Federation in connection with the incoming requests for transfer ambulance on the outsourcing of information was published as follows: “the Ministry of health of the Russian Federation proceeds from the fact that to transmit any non-governmental organizations, the very provision of emergency medical care is unacceptable. Such a scenario is not considered and not discussed.

Simultaneously, the Ministry of health of Russia together with other Federal authorities studying the experience of the transfer of transport functions exclusively on outsourcing. This experience is in a number of regions. It is analyzed and discussed with experts. If he is found effective, the appropriate model can be proposed to the regions for implementation. However, about any decision-making process may not be considered

The Ministry of health of the Russian Federation has not supported initiatives to establish limits on the number of free calls for medical assistance. We believe that the right to receive ambulance is basic and cannot be limited by some sort of economic framework.

The Ministry of health has consistently improved approaches to the organization of emergency medical care in the country. The main goal is to increase its availability to the public. So, the Ministry has issued an order dated February 27, 2016 No. 132n “On requirements to the placement of the medical organizations of the public health system based on the needs of the population.” According to the document, the location of ambulance stations, as well as other departments providing ambulance, should be carried out taking into account the time of arrival of ambulance crews to the most distant settlement of the territory served, not to exceed 20 minutes from the time of its challenge,” the article says.
